A free online Advance Directive Workshop for all Patients
This is a 2-part workshop: Wednesdays, November 4th and November 18th, 12:00 pm - 2:00 pm. Please plan to attend both sessions.

Presented by the MERI Center for Education in Palliative Care at Mount Zion.

This online two-part workshop enables you to:

• Create a personalized plan to honor your wishes
• Have your questions answered
• Complete your new or revised Advance Directive for Healthcare to be notarized for free

There is no fee for this workshop, and it is open to non-UCSF patients, as well as UCSF patients.
