PREPARE Workshop on Advance Care Planning

Advance care planning is a process that supports adults at any age or any stage of health in understanding and sharing their personal values, life goals and preferences regarding current and future medical care. The goal is to help ensure that people receive medical care that is consistent with their values, goals, and preferences during serious and chronic illness.

The PREPARE Workshop on Advance Care Planning is a step-by step program that will provide you with tools to help you:

• identify future hopes and wishes for medical care;
• select someone who will speak for you if you cannot; and
• learn how to better talk with doctors, family and friends about your wishes.
