Objectives:

  • Recognize how serious medical errors and unintended harm to patients can occur even in excellent health care settings (in-patient, out-patient, and cyber space).
  • Adopt constructive approaches to avoid human error and reduce medical errors including diagnostic errors.
  • Describe how clinical decision support tools in an electronic health record can be used to mistake-proof certain clinical workflows.
